Palliative home care

Palliative care at home, when comfort matters most.

When a loved one is approaching the end of life, families want symptoms managed gently and a doctor reachable when things change — without another trip to hospital. We provide home visits for symptom relief and after-hours support, working alongside your hospice or palliative team.

When families reach for us

The hardest moments often come after hours.

Symptoms rarely keep office hours. Pain builds in the evening; breathing becomes laboured overnight; agitation or nausea arrives when the regular team is not contactable. In those moments, an ambulance to A&E is often the last thing a family wants for someone who has chosen to be at home.

A doctor — including emergency-medicine specialists experienced in acute symptom control — can come to the home, assess what is happening and help settle symptoms there, so your loved one stays where they are most comfortable. When hospital truly is the right place, we help you get there.

How we help

Specialist support, gently delivered.

Symptom relief

Assessment and management of pain, breathlessness, nausea, agitation and other symptoms at home, including medication by injection where needed.

After-hours response

A doctor reachable through the night, weekends and public holidays — for the moments between your team's scheduled visits.

Family guidance & coordination

Time to explain what is happening, what to expect, and a written summary shared with your hospice team and GP, with your consent.

Alongside your team

We complement hospice care — we don't replace it.

Singapore has excellent hospice and home-palliative services, and many families are already under their care. We are not a substitute for them. We are the doctor you can reach for an after-hours symptom change or an unplanned home visit, sharing what we find so your existing team stays in the picture.

When the time comes, a doctor can also attend to certify a death at home — unhurried, with the documentation handled for the family — so there is no need to move your loved one or wait alone.
